Submission Requirements

• Thomco Med Lab Application (Click Here to apply online)
• Four year, hard copy loss runs
• Current CLIA Certificate
• Copy of states inspections
• Current, audited financial statements
• Contracts with independent contractor physicians and labs
• Most recent local and/or state accreditation agency reports (CAP, JCAHO, etc.)
